Initial Connection
We start with a conversation. This may come through self-referral, court involvement, or a partner agency. We take time to understand the individual’s current situation, needs, and goals.

Assessment & Goal Setting
We identify strengths, barriers, and immediate priorities such as housing, employment, legal obligations, and stability. Together, we create a clear, realistic plan.

Individualized Plan Development
Each person receives a structured reentry and stability plan tailored to their life circumstances. This includes actionable steps, timelines, and accountability measures.

Ongoing Support & Accountability
We provide consistent support through 1:1 sessions, coaching, and check-ins. Individuals are held accountable in a way that promotes growth—not shame—while building confidence and follow-through.
